Privacy Policy
Effective September 15, 2026
1. Who we are
Six Equation (SEQ) Medical Billing Services (“SEQ”, “we”, “us”) provides revenue cycle management services to healthcare providers. Our office is at 1881 N University Dr, Suite 105, Coral Springs, FL 33071.
This policy explains what we do with information collected through this website, sixequation.com, and when you contact us or book an appointment with us.
2. What this policy does not cover
When we provide billing services to a healthcare provider, we act as a Business Associate under the Health Insurance Portability and Accountability Act (HIPAA). Protected health information we handle in that role is governed by HIPAA and by our Business Associate Agreement with that provider — not by this website policy.
If you are a patient with a question about a medical bill, a statement, or your health records, please contact your healthcare provider directly. They are the covered entity responsible for your information. Please do not send patient information, medical records or account details to us through this website, by email, or through our booking page — none of those channels are secured for that purpose. Call us at (800) 992-9979 instead.
3. Information we collect
When you browse this website. This site has no contact form, no user accounts, and sets no analytics or advertising cookies. Our web host keeps standard server logs — IP address, browser type, pages requested and timestamps — which are used for security and to keep the site running.
When you book an appointment. Our booking page is hosted by Microsoft Bookings, part of Microsoft 365. If you book a call, we receive the details you enter: your name, email address, phone number, and anything you write in the other fields. This is stored in our Microsoft 365 environment.
When you call or email us. We keep the contents of your message and our reply, along with your contact details, so we can respond and maintain a record of the conversation.
Fonts. This site loads its typeface from Google Fonts. When a page loads, your browser requests those font files from Google, which makes your IP address visible to Google. We receive nothing from this and use it for nothing.
4. How we use this information
- To respond to your inquiry and to hold the appointment you booked
- To provide, administer and invoice for our services
- To keep our systems and our clients' data secure
- To meet our legal, regulatory and contractual obligations
We do not sell personal information. We do not share it with advertisers, and we do not use it to target advertising to you.
5. Who we share it with
We share information only where it is necessary, and only with:
- Service providers who operate systems on our behalf — our web host, and Microsoft for email, scheduling and file storage. They may process information only on our instructions.
- Professional advisors such as our accountants and lawyers, where relevant.
- Authorities, where we are required to by law, court order, or to protect our legal rights.
If we were ever involved in a merger, acquisition or sale of assets, information could transfer as part of that transaction. We would say so before it happened.
6. Cookies
This website does not set cookies and does not use tracking or analytics scripts. Our booking page is operated by Microsoft and may set its own cookies; Microsoft's privacy statement governs that page.
7. How we protect information
We are HIPAA compliant and operate an Information Security Management System built on the confidentiality, availability and integrity of data. Controls include staff verification, biometric access control at entry and exit points, video surveillance, restrictions on portable devices in production areas, servers located in the United States, and VPN access to client systems. Security tooling is maintained by qualified personnel.
No method of transmission or storage is completely secure, and we cannot guarantee absolute security. Email in particular is not a secure channel.
8. How long we keep it
We keep inquiry and appointment information for as long as needed to respond to you and to maintain a reasonable business record, and then delete it. Information held under a client contract is retained for the period set out in that contract and in the applicable Business Associate Agreement, and is returned or destroyed at the end of the engagement as those agreements require.
9. Your choices
You may ask us to provide a copy of the personal information we hold about you, correct it if it is wrong, or delete it. Write to info@sixequation.com and we will respond within a reasonable period. We may need to verify your identity first.
Requests about health information held on behalf of a healthcare provider must go to that provider, not to us — as a Business Associate we are not permitted to act on those directly.
If you are a California resident, you have additional rights under the California Consumer Privacy Act, including the right to know what personal information we collect and the right to request deletion. We do not sell personal information, so there is nothing to opt out of. Use the same address above to make a request.
10. Children
This website is intended for healthcare providers and their staff. It is not directed at children, and we do not knowingly collect personal information from children through it.
11. Links to other sites
This site links to our booking page, which is operated by Microsoft. We are not responsible for the privacy practices of sites we do not operate.
12. Changes to this policy
We may update this policy from time to time. The effective date at the top of this page shows when it last changed. Material changes will be posted here.
